Injury surveillance and outcome management of stabbings in the ED.
The most common cause of death in Australia is stab wounds. The aim of this study was to determine the injury characteristics and management of patients with stab wounds in the ED. Over two years, all consecutive adult patients with stab wounds to the thorax, abdomen, or extremities presenting to the ED of a major trauma centre were studied. Outcomes were compared with the American College of Surgeons definition of severe trauma. Two hundred and twenty-three patients were studied (64% male). Seventy-nine percent sustained injuries to the extremities. Ninety-one percent had penetrating wounds. The mean Abbreviated Injury Scale (AIS) score for patients sustaining penetrating torso wounds was significantly higher than that of patients sustaining penetrating wounds to the extremities (2.2 vs 1.4; p = 0.004). The AIS was comparable for patients sustaining penetrating wounds to the thorax or abdomen (1.9 vs 1.6, p = 0.5). Of the 183 patients with penetrating wounds to the extremities, 51 (28%) required transfusion of blood products, 32 (18%) required intubation and 21 (12%) were admitted to the intensive care unit. Twenty-nine (13%) had complications, 25 (12%) died, and 41 (22%) had a resultant permanent disability. Fifty-five (25%) patients had an AIS score less than 3. Injury characteristics of patients sustaining stab wounds to the extremities were comparable to those sustaining stab wounds to the torso. The overall success of the application of the American College of
